Our understanding of the electrocardiogram in the child has expanded even in the last five years, advancing our diagnostic capability in certain areas and defining our limitations more realistically in other aspects of the field. It has become necessary to reassess our knowledge of what we considered to be fundamental concepts as well as the application of these principles in the interpretation of the clinical elec-trocardiogram. This book is entitled "A Systematic Approach" because it presents the principles of electrocardiography sequentially, in an orderly man-ner. It is meant to be read from cover to cover, since much of the information contained in the later chapters builds upon concepts elaborated earlier. Since most of us learn best by the Socratic method, self-assessment questions are included at the end of each chapter with a discussion of the answers. The first half of the book is devoted to the normal electrocardio-gram: electrophysiologic principles, summation of cellular events on the body surface, definition and measurement of the components of the electrocardiogram, and the variations produced solely by malpo-sition of the heart within the thorax. The second half of the book concerns the abnormal electrocardiogram and its value in diagnosing hypertrophy, ischemia, and infarction, considering the recent advances in echocardiography and nuclear imaging. In the chapter on arrhythmias, two approaches are taken: in the electrophysiologic sec-tion, the major arrhythmias are discussed according to their site of origin and mechanism; in the morphologic section, a stepwise method of rhythm analysis is presented. Finally, a systematic approach to the recognition of artifacts is presented in the hope that the child with a normal heart will not become a victim of faulty interpretation of a normal electrocardiogram.
